If you choose to use the Pale Dagger to cut a heart free from a dead Worm Cultist, Lieutenant Belron will instruct you to put the heart into the Worm Nightfrenzy's chest. She will rise, staggering, and you may interrogate her. She will reveal that the Reachmen are being blessed with the power of Molag Bal and Stral himself is undergoing a transformation:

"I can feel my flesh decay. But I'm not dead. Please, kill me. I'll tell you anything if you promise to kill me."

Tell me what I want to know first.

"We made a deal with the Reachmen. They unleashed the giant, Sinmur. And we promised to bless them with the touch of our dark lord."
